# When is lumbar stenosis severe? Criteria for considering surgery

Lumbar stenosis is considered severe when it significantly limits daily life or when it affects nerve function, not just because of what the MRI shows. The criteria that lead to considering surgery are: claudication that greatly reduces the distance you can walk (stopping every few metres), pain that does not respond after months of well-conducted conservative treatment, loss of strength in the legs or foot, and, in advanced cases, difficulty urinating or with bowel movements. The degree of narrowing on imaging does not always match the clinical severity: the decision is made by combining the MRI with the real impact on the patient. When these criteria are met, decompression surgery usually markedly improves the ability to walk and quality of life.

## "Severe" is clinical, not just radiological

An MRI may show a very narrow canal in someone with few symptoms, or moderate narrowing in someone with major limitation. What defines severity is the impact on the person: how far they can walk, whether there is loss of strength and how much it affects their daily life.

## Criteria that tip towards surgery

- Claudication that greatly reduces walking distance (stopping every few metres).
